"client_send" finde ich im Handbuch gar nicht ;-) Vielleicht hilft Dir
folgendes (shameless plug von

cram:
  driver = cram_md5
  public_name = CRAM-MD5
  server_secret = ${if eq{$1}{ph10}{secret1}fail}
  client_name = ph10
  client_secret = secret2

Übrigens sollte man laut Handbuch nicht mehrere Male denselben
Authenticator konfigurieren. Du hast zweimal login und zweimal plain
drinnen ;-)
